Review new or replacement Bridge, Culvert or Wall serial number requests with the Bridge Squad Leader assigned to the project, then fill out the appropriate Serial Number Request Form (found at the links above) and press the "E-mail Form" button to submit the Serial Number Request Form to:

You will receive a Serial Number Assignment Memo by reply e-mail. The Bridge MSA will send a copy of the Serial Number Assignment memo to the Bridge Squad Leader assigned to the project.
